What Is CS Form 212?

  • Learning & development activities - Indicate details of the learning and development programs or the training you have taken in the last five years which are relevant to the job position you are applying to.
  • Other information - In this section, you need to give the particulars of any recognized skills or hobbies you have that are relevant to your job application.
  • Indicate yes or no if you are related in any way to the authority of the job position you are applying to.
  • Criminal cases - Details of any criminal offenses, court cases, convictions of law violation, and other criminal records against you.
  • Service terminations - Information about your previous service terminations in an unprecedented way or for election campaigns is to be indicated.
  • References - A reference person needs to vouch for the information you have provided who are not related to you by consanguinity or affinity.
  • Documents to be included - Passport sized photograph and a government-issued ID card.
